Release Of Claims Personal Injury and Assumption of Risk

In consideration of your acceptance of my application, I do hereby, for myself, my heirs, executors and administrators waive, release and forever discharge any and all rights and claims for damages which I may have, or which may accrue to me, against U.S Junior Taekwondo, Inc., Azusa Pacific University, and its Tournament Organizing Committee, and all members of the championship, or their respective officers, agents, representatives, successors, and/or assigns, and against any competitor for any and all damages which may be sustained by me in connection with my association with or entry in the above athletic meet, and in connection with any medical services I may be provided in connection with such injury or illness. I understand that the competition rules are based on the rules of the WT. I agree to all the tournament competition rules and understand that if I do not follow these rules, I knowingly forfeit all applicable registration fees. I further understand that any pictures taken of me in connection U.S. JUNIOR TAEKWONDO CHAMPIONSHIP  may be used by the Tournament Director for publicity or promotion without compensation. I understand that I have the sole responsibility to attend this event, any absence can not be compensated (referred to "Refund Policy"). I hereby agree to all the terms and conditions of the liability waiver above.  I understand that in case of injury, only basic first aid will be made available on site, and that I am fully responsible for any and all resulting medical or other expenses.

I understand that there is an inherent risk of exposure to COVID-19 and other susceptible viruses by entering APU's facilities and attending any event, program, practice and/or competition (hereinafter referred to as the “Activity”). Participating in such Activity could increase my risk of contracting COVID-19 and other susceptible viruses. I further understand that the U.S Jr. Taekwondo, Inc. is not responsible for, and cannot adequately evaluate the present health conditions or health risks of all persons present during the Activity at APU's  facilities. By attending the U.S Jr. Taekwondo Championship on the College’s campus, I am voluntarily assuming all risks of exposure to, infection from or transmission of COVID-19 and other susceptible viruses.

Refund Policy

After your application and payment is received and accepted by the US JR Taekwondo, Inc., payments are non-refundable.

Admission Tickets are non-refundable 7 days before event day. Tickets bought at the Door are non-refundable.
